Cancellation Policy

Cancellation/ Refund Policy: Two business days before class start date 100% refund of any amount above the initial deposited amount. Refund request must be made in person and are issued within 30 days of cancellation date and will be sent via US mail. If student withdraws or is withdrawn, more than two (2) days to class start date, administration fee will be deducted from total refund. If withdraw occurs less than two (2) days prior, student will receive 0% of refund. I acknowledge by my signature below that: A. I have received, read, and agree with Tailored Care, LLC policies and procedures as written; B. Meet all admission requirements as outlined in the policies and procedure of Tailored Care, LLC; C. I agree to submit payment for payment for tuition and fees as outlined above and that this document becomes a legal and binding contract once completed and signed by both parties; D. Financial obligation of the program has been explained to me; E. I do not have any physical or mental limitations that would jeopardize the resident, myself, faculty, or staff to any harm.
